Based on my reading of the publisher's web site, it is now OK to post preprints on the author's web site.
Thus I'm making available the latest pre-publication draft I could find as Postscript and PDF, particularly since electronic copies of this paper are otherwise difficult to obtain.
Note that this is not the final version of the paper. It was generated from 16-year old troff sources, and may have formatting glitches. And things happened in the 16 years since it was published.
